Patients with HMO plans or Tricare Prime should call their Insurance to select Dr. Farah as the primary care physician before the first appointment with us.
New patients are advised to arrive ten minutes prior to appointment time. Bring a photo ID and your Insurance card. Please note that co-pays are due upon your visit. We accept credit cards, cash & checks.
Your time is very important to us! We do not double book so your waiting time is kept to the minimum possible. If you cannot make it for your appointment, please give us twenty-four hours advanced notice so we can give that appointment to another patient who needs it. There is a charge for no-shows.
